đŸ©șInternal palpation for examination and treatment is the most daunting skill to learn when first starting out as a pelvic health practitioner.
One thing I have to remind my student’s is that as pelvic health therapists we are musculoskeletal experts; therefore, orient yourself to where you are most familiar with - the bone. 🩮
While internal, you should not be in “no man’s land” aimlessly wondering around.
You should be finding bony landmarks to orient what you are doing and what needs to be treated.
Here are some examples:
Coccyx: 6 o’clock: Levator ani and coccygeus.
Hip bone: 3 & 9 o’clock: Obturator internus
Pubic bone: 11-1 o’clock: Peri-urethral and urethra
Pubic ramus: 1-3 & 10-11 o’clock: Ischiocavernosus & anterior fibers of obturator internus

Hey there! 👋 Did you know that understanding the interconnections of muscles in the male body can help with urinary issues? đŸ€” A 2020 study analyzed Japanese male cadavers and found some surprising findings. For instance, the external anal sphincter had anterior connections to the bulbospongiosis, a muscle halfway up the shaft of the penis, instead of the perineal body. Additionally, the levator ani muscle not only covers the outer surface of the external anal sphincter but also connects to other muscles. Including the anterior muscle fiber bundles that had attachments to the external urethral sphincter and bulbospongiosus! This shows the interdependency of muscles in the male body, which is crucial for understanding and treating urinary issues comprehensively. We can't just focus on one area but must consider the big picture for our patients.
