đŸ©șInternal palpation for examination and treatment is the most daunting skill to learn when first starting out as a pelvic health practitioner.
One thing I have to remind my student’s is that as pelvic health therapists we are musculoskeletal experts; therefore, orient yourself to where you are most familiar with - the bone. 🩮
While internal, you should not be in “no man’s land” aimlessly wondering around.
You should be finding bony landmarks to orient what you are doing and what needs to be treated.
Here are some examples:
Coccyx: 6 o’clock: Levator ani and coccygeus.
Hip bone: 3 & 9 o’clock: Obturator internus
Pubic bone: 11-1 o’clock: Peri-urethral and urethra
Pubic ramus: 1-3 & 10-11 o’clock: Ischiocavernosus & anterior fibers of obturator internus

👀 Why does a kick in the balls cause such intense pain for men? Why is the immediate reaction is to draw in and recoil? đŸ€” One key player is.....drumroll....the Dartos muscle! The dartos muscle is so cool. It is not striated like the cremaster muscle, but is a smooth muscle (involuntary) intricately connected to the external oblique muscle and is innervated by the sympathetic nervous system. Our flight, fight and freeze center! Because of those 2 influences it makes sense why you would go into this immediate "sympathetic" posture. Shorten the abdominal muscles, go into a protective mode. Sympathetic innervation for the pelvis originates in the lower thoracic and upper lumbar. So, T/L junction, abdominal region, external genitalia...it is all connected.
